Independent Driver Service Agreement and Obligation

Upon accepting you are agreeing to the following items that will be conducted and carried out by Compliance Safety Systems:
1. Your credit card will be drafted for the agreed amount of, , and will be drafted on the 1st of each month.
2. As required in 391.2 CSS will make an inquiry into your driving record during proceeding three years to the appropriate state agency(s) and an investigation of your employment record during the preceeding three years (391.23) and will perform annual review of your driving record (391.25) or furnish a record of violations or a certificate(391.27).
3. CSS will obtain an FMCSA "Portal" user id and password to the US DOT can be monitored using the Motor Carrier Management Information System when determining if a Roadside Inspection was conducted and it is legitimate.
4. CSS will obtain a "Pre-Screening Program" URL and pin id so any information that is recorded thru the MCMIS report for the previous 36 months can be reviewed and reported to the driver and or owner operator, so information can be corrected if not correct and a ranking/scoring system can be determined should new employment or leasing contracts be sought in the future or within the current contractor or employer.
5. CSS will obtain a "Data Q" user id and password so any Roadside Inspection Report can be challenged should the Driver NOT agree with the inspection report.
Compliance Safety Systems will perform the following services.
1. Gather the required documents that will pertain to any roadside inspection, such as the drivers Medical Card, I-9 information, copy of the driver license, etc and image those documents into a "Dashboard" that is made available 24/7 to the driver via the CSS website.
2. CSS will monitor, record and display the Motor Carrier Management Information System Roadside Inspection Data to the Owner Operator or US DOT holder. Those reports will be communicated 24/7 to the driver and consultaion will be discussed with the driver should activity by present.
3. CSS will aid the Driver with any Data Q should the inspection report reflect and discrepancies or errors that the driver does not agree with. (the driver MUST be prepared to issue and documents needed to dispute the isnpection report, so the States can view the documentation that supports the dispute).
4. CSS will inquire and request a PSP report, (pre-screening program) that reflects any roadside data history so consultaion can be discussed with the driver regarding any items needing improved upon or disputed.
5. CSS will build the drivers own qualification and performance file so the driver will always know where they stand when it comes to understand what is being said about the driver during the coarse od employment or seeking new employment or broker agreements.
Foot Note: If you are an independent driver and DO NOT have a US DOT number because you are an employed driver, then it will be your responsibility to report any and all Roadside Inspection to your Dashboard via by faxing or emailing the actual report to CSS, so your file is always up to date and you will always know what is being said about you in the PSP model.
